MDSQ Options counseling

The MDSQ Options Counselor provides individuals in skilled nursing facilities, their families, and their caregivers with information about services and supports needed to return to the community and assists by making referrals to available services based on identified needs and eligibility.    

After being in a nursing facility, it can be difficult for individuals to know how to start the process if they wish to return home.  Certified MDSQ-Options Counselors can help with this process.  An Options Counselor will talk with the person in the facility about what is important to them and provide information they need to make informed choices about returning to the community.  The Options Counselor will also make referrals based on services the person may be eligible for and will help to explain Medicaid, non-Medicaid, and private pay service options.  Anyone can make an Options Counseling referral for someone in a nursing facility that wishes to return home by contacting their local Area Agency on Aging or by contacting the MDSQ-Options Counselor that serves their local county.

Eligibility Requirements

  • Minimum of 60 consecutive day stay in a nursing facility

  • Receive Nursing Home Medicaid
